Hazard Assessment Guide Table

The Hazard Assessment Guide Table should be filled out before a new experimental facility begins operation, and once a year during the Self-Assessment process by all EETD facilities.

Activity Hazard Document (AHD) Form

This table will help you determine whether authorizations (such as an AHD) are required for the hazards in your facility.

• Class 3b and Class 4 lasers
• Other high hazards, such as toxic or reactive chemicals, high pressures, etc. [Download]

If you need an AHD, the EH&S Division staff will assist with the document. Contact your Division Safety Coordinator and/or the EH&S Division Liaison.

Specific Safety Procedure (SSP) Form

Useful for chemical hazards or other procedures needing safety documentation but not requiring an AHD or other authorization.
